Important Note: Passports are processed by the U.S. Department of State. The Office of the City Clerk performs in-person application review and acceptance before forwarding your documents to the U.S. Department of State for processing.

 

The Office of the City Clerk is experienced with complex applications (e.g., delayed birth, birth abroad). To avoid delays, please arrive 10 minutes prior to your appointment. Please complete and print a Passport Application (Form DS-11) prior to your visit. Filling the form out by hand? Please use a black ink pen.


Adult Passport Application and Renewal Requirements

  1. Proof of US citizenship
    • Birth certificate showing birth in the United States, (must include full name of parent); or
    • Certificate of Naturalization (Form N-550); or
    • Certificate of Citizenship (Form N-560); or
    • Report of Birth Abroad of United States Citizen (Form FS-240); or
    • Valid unexpired U.S. passport.

  2. Valid photo ID

  3.  Passport photos (can be purchased at our offices)

  4. Application (Form DS-11)

  5. One check or money order payable to the US Department of State per application (see table below to determine amount)


Minor Passport Application and Renewal Requirements
(newborn to 15 years old)

  1. Both parents and child must be present to process application 
    • One parent and the child may appear and present documentation of sole legal guardianship
    • One parent and the child may appear with an original Statement of Consent (Form DS-3053) signed by the absent parent and notarized, as well as a copy of the absent parent’s valid ID front and back.   

  2. Proof of US citizenship
    • Birth certificate showing birth in the United States, (must include full name of parent(s)); 
    AND if available
    • Certificate of Naturalization (Form N-550); or
    • Certificate of Citizenship (Form N-560); or
    • Form FS-240, Report of Birth Abroad of United States Citizen; or
    • Valid unexpired U.S. passport.

  3. Valid photo ID of both parents

  4. Passport photos (can be purchased at our offices)

  5. Application (Form DS-11)

**Passport cards are only valid if you plan to travel into the U.S. by land or sea from Canada, Mexico the Caribbean, and Bermuda. If you plan to fly, you must obtain a Passport Book for reentry to the US. 


Are there any other fees for getting a passport?

There is a $35.00 processing fee per application, payable to the City of Henderson. (This fee can be paid as debit, cash, check, or money order.)

In addition, you can have passport photos taken at our office for $12.00 per application. (This fee can be paid as debit, cash, check, or money order.)


Passport Book vs. Passport Card

The U.S. passport card facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. It will otherwise carry the rights and privileges of the U.S. passport book and will be adjudicated to the exact same standards.        

A passport book is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid U.S. passport is required to enter and leave most foreign countries. Only the U.S. Department of State has the authority to grant, issue, or verify United States Passports.


Passport Photo Requirements

  • Pupils and irises must be clearly visible

  • Full face must be visible and free of shadows

  • Hair must not obscure the face

  • Glasses must be removed even if glasses are routinely worn

  • Uniforms, such as military or police, must not be worn

  • Religious clothing worn daily may be worn, but must not obscure the face

  • No hats or head coverings, except for religious purposes. A signed statement regarding religious headgear must be requested

  • Photo must not show headphones or a wireless device, such us Bluetooth headset or earbuds

We are in City of Henderson City Hall at 240 S. Water St., Henderson NV, 89015.

Parking is available off Basic Road. Overflow parking is available in the Water Street parking garage across from City Hall.
