No appointment necessary. If you have a specific question on a claim, please bring your Explanation of Benefits (EOB) form. If you don't have an EOB, please bring the date of service and name of the provider.
A UMR representative will be scheduled for onsite visits every second Wednesday of every month to answer questions regarding your medical and dental claims. UMR is responsible for processing and paying health claims based on the Self-Funded Plan guidelines.
